Join Mindoula Health's team as a Care Manager in their Teen Suicide Prevention Program in Texas! This full-time position offers a flexible schedule with remote work options and involves providing virtual and in-person support to individuals and families affected by suicide. You will engage with members, coordinate care, manage crises, develop service plans, educate, and advocate. The role requires a Bachelor's degree in a behavioral health field, crisis intervention training, and at least two years of relevant experience. Requirements
- Bachelor’s degree in psychology, social work, or another applicable behavioral health discipline from an accredited institution
- At least 2 years of experience in a related role, preferably working with at-risk adolescents and young adults
- Crisis intervention training or similar certification is required
- Knowledge of local resources, social determinants of health, mental health, substance abuse disorders, interpersonal violence, suicide ideation and social issues
Responsibilities
- Assist with outreach, engagement, and continuous support for a cohort of members, providing regular check-ins and personalized care
- Develop and coordinate community resources to address medical, behavioral, violence prevention, and social service needs
- Assess risk, perform crisis interventions, and ensure safety through tailored safety plans
- Collaborate with members to assess care options and develop and update service plans, including leveraging benefits and community resources
- Educate members about mental and physical health, abuse prevention, medication, and available resources. Serve as an advocate between members, families, and providers
- Assist in scheduling appointments and follow-ups, ensuring members attend their sessions and appointments
- Maintain accurate and up-to-date records of all case management activities, documenting case notes in a professional, timely manner
