Join Salvo Health as a Care Manager Supervisor (RN) and oversee the daily operations of the care navigation team, ensuring high-quality patient care and compliance with standards. You will supervise care navigators, coordinate interdisciplinary care, and implement strategies to improve patient outcomes. This role involves a balance of patient care (25-50%) and team management (50-75%). Responsibilities include managing a team of 10-15 nurses and medical assistants, conducting clinical intake visits, and implementing workflow enhancements. The ideal candidate possesses an active RN license in an NLC state, 2+ years of experience managing nurses in a virtual setting, and 2+ years of clinical experience with gastroenterology patients. A Bachelor's or Associate's degree in nursing is required.
Requirements
- Active RN license in a Nursing Licensure Compact (NLC) state and eligible for licensure in all U.S. states
- 2+ years of experience managing nurses, ideally in a virtual setting
- 2+ years of clinical experience working with gastroenterology patients, with a strong interest and knowledge of GI conditions
- Associate or Bachelor's degree in Nursing from an accredited institution
Responsibilities
- Build and manage a team of 10-15 outstanding nurses and medical assistants, including managing QA/QI, supervision, scheduling, and working with our administrative team to manage licensure of team members
- Be a player coach: spend approximately 25-50% seeing patients and half your time managing the team
- Conduct clinical intake visits with prospective patients via video and/or phone. Intakes include verifying and documenting a clinical history, verifying medications, answering basic questions about the Salvo Health program, and setting them up for success in the program
- Commitment to providing care of the highest quality that delivers an exceptional experience for the patient
- Daily Schedule hygiene for assigned nursing team to enhance patient throughput
- Providing updates to clinical intake workflow
- Delivery of just in time feedback as needed
- Weekly Care Manager meeting with team
- Working with Clinical Operations to suggest and implement workflow enhancements
- Handling patient escalations to ensure timely and appropriate care coordination is provided

Benefits
$70,000 - $75,000 a year
