Summary

Join Salvo Health as a Care Navigation Supervisor (RN) and oversee the daily operations of a care navigation team, ensuring high-quality patient care and compliance with standards. You will supervise care navigators, coordinate interdisciplinary care, and implement strategies to improve patient outcomes. This role involves managing a team of 10-15 nurses and medical assistants, including QA/QI, supervision, scheduling, and managing team licensure. Approximately half your time will be spent seeing patients, while the other half focuses on team management. Conduct clinical intake visits with prospective patients, verifying clinical history, medications, and answering program-related questions. Responsibilities

  • Oversee the daily operations of the care navigation team, ensuring the delivery of high-quality patient care and compliance with healthcare quality and documentation standards
  • Supervise care navigators
  • Coordinate interdisciplinary care
  • Implement strategies to improve patient outcomes
  • Build and manage a team of 10-15 outstanding nurses and medical assistants, including managing QA/QI, supervision, scheduling, and working with our administrative team to manage licensure of team members
  • Spend approximately 25-50% seeing patients and half your time managing the team
  • Conduct clinical intake visits with prospective patients via video and/or phone
  • Verify and document a clinical history during intake visits
  • Verify medications during intake visits
  • Answer basic questions about the Salvo Health program during intake visits
  • Set up patients for success in the program during intake visits
  • Provide commitment to providing care of the highest quality that delivers an exceptional experience for the patient
  • Manage daily Schedule hygiene for assigned nursing team to enhance patient throughput
  • Provide updates to clinical intake workflow
  • Deliver just in time feedback as needed
  • Conduct Weekly Care Manager meeting with team
  • Work with Clinical Operations to suggest and implement workflow enhancements
  • Handle patient escalations to ensure timely and appropriate care coordination is provided
