Clinical Navigation, Family Nurse Practitioner

Summary

Join Included Health as a 100% Remote Family Nurse Practitioner, working per diem hours within PST or MST time zones. You will provide virtual care to patients, assessing their needs, offering clinical guidance, and connecting them with appropriate resources. This role involves collaborating with a multidisciplinary team to optimize patient outcomes and requires at least 3 years of NP/PA experience. Included Health offers a fee-for-service model with full coverage for no-shows and late cancellations, along with comprehensive administrative support. Requirements

• Be a licensed NP/PA provider in good standing with your state board
• Maintain board certification
• At least 3 years of NP/PA experience in a clinical setting is required
• Comfortable providing care to all age groups, from newborns to adults
• Ability to thrive in a dynamic work environment while working independently
• Must be in PST or MST time zone

Responsibilities

• Talk with patients, assess their needs, gather important clinical history, provide appropriate clinical guidance, and navigate them to Included Health services, external benefits, and local resources as appropriate
• Demonstrate strong clinical acumen across a variety of Included Health clinical services, including Expert Medical Opinion, Treatment Decision Support, Healthy Days, and other service lines based on business needs
• Maintain Quality of Services: Deliver high-quality clinical services that follow established standards and protocols. Regularly monitor and assess the quality of care provided to ensure it meets or exceeds patient satisfaction and clinical effectiveness benchmarks
• Evaluate, triage and manage patients across patients for both acute, chronic and mental health conditions
• Collaborate on pilots, projects, and workflow changes to increase efficiency and improve member experience

Preferred Qualifications

• 5+ years of NP/PA experience in a clinical setting is preferred
• Virtual care/telemedicine experience is preferred
• Multi state licensed preferred
• Bilingual is preferred
