Clinical Navigation, Family Nurse Practitioner

Summary

Join Included Health as a 100% Remote Family Nurse Practitioner and help redefine healthcare by providing virtual care to patients. This per diem role, under the guidance of a Clinical Navigation Manager, allows you to set your own hours within a flexible schedule. You will assess patient needs, provide clinical guidance, and navigate members to resources. Collaborate with a multidisciplinary team to optimize patient outcomes and contribute to projects aimed at increasing efficiency and improving member experience. The position requires a current license, board certification, and at least 3 years of NP/PA experience. Included Health offers a fee-for-service model with full coverage for no-shows and late cancellations, along with comprehensive administrative support.

Requirements

• Be a licensed NP/PA provider in good standing with your state board
• Maintain board certification
• At least 3 years of NP/PA experience in a clinical setting is required
• Comfortable providing care to all age groups, from newborns to adults
• Ability to thrive in a dynamic work environment while working independently
• Must be in PST or MST time zone

Responsibilities

• Talk with patients, assess their needs, gather important clinical history, provide appropriate clinical guidance, and navigate them to Included Health services, external benefits, and local resources as appropriate
• Demonstrate strong clinical acumen across a variety of Included Health clinical services, including Expert Medical Opinion, Treatment Decision Support, Healthy Days, and other service lines based on business needs
• Maintain Quality of Services: Deliver high-quality clinical services that follow established standards and protocols. Regularly monitor and assess the quality of care provided to ensure it meets or exceeds patient satisfaction and clinical effectiveness benchmarks
• Evaluate, triage and manage patients across patients for both acute, chronic and mental health conditions
• Collaborate on pilots, projects, and workflow changes to increase efficiency and improve member experience

Preferred Qualifications

• 5+ years of NP/PA experience in a clinical setting is preferred
• Virtual care/telemedicine experience is preferred
• Multi state licensed preferred
• Bilingual is preferred
