Clinical Pharmacist

Summary

Join a dynamic team as a Clinical Pharmacist and help members on their journey to healthier hearts. In this role, you will evaluate patient charts, identify prescription medication treatment gaps, and empower patients to speak with their prescribing provider to close gaps that exist.

Requirements

• Doctor of Pharmacy (Pharm.D.) from an accredited school of pharmacy, preferably with Board Certification in Pharmacotherapy (BCPS) or Ambulatory Care (BCACP)
• Minimum of 2+ years of clinical pharmacy experience, preferably within telehealth
• Actively maintains multistate pharmacist licensure
• Strong knowledge of pharmacology, clinical guidelines, and best practices in medication therapy management
• Familiarity with virtual patient care platforms and protocols
• Understanding of patient data privacy laws (e.g., HIPAA) and secure handling of electronic medical records
• Ability to work independently and collaboratively, as required, in a fast-paced, start-up environment

Responsibilities

• Review patient medical records to determine if guidelines-based prescription medication gaps exist
• Communicate with patients about prescription medications gaps that should be closed
• Collaborate with healthcare providers (e.g., physicians, nurses) to recommend medication therapies based on clinical guidelines and patient-specific data
• Maintain detailed records of all consultations and communications with patients, including up-to-date electronic health records (EHRs)
• Stay informed on latest healthcare practices, telehealth technologies, and continuing education requirements
• Help connect patients and family members to community resources, support groups, or other services they may need
