Requirements

  • High school diploma or GED
  • CHW certification required from approved Community Health Certification Program, or two years of equivalent work experience
  • 1+ years experience care coordination and navigation

Responsibilities

  • Assist members in following an outcome-based plan of care, based on the member's input and assessment results and identify/address current needs of the member, using outlined escalation pathways when needed
  • Outreach identified members to identify and help solve for barriers that may affect their care by phone outreach, secure messaging and mail
  • Monitor care and services provided to members to ensure that quality care is being delivered in the most appropriate setting
  • Ensure the "Plan of Care" moves with the member and update the care plan as the member's health status changes
  • Assist members by providing resources, education and information promptly
  • Compliance with all applicable laws and regulations
  • Other duties as assigned

Preferred Qualifications

  • Bilingual [English and Spanish or Creole]
  • AA or Bachelor's degree in social science or health related field
  • Behavioral Health Experience
