Community Health Worker

Summary

Join Oscar's Case Management team as a Community Health Worker! Support members in achieving chronic disease self-management goals by providing resources and education. You will conduct outreach, monitor care, and update care plans based on member needs. This role offers a blended work culture with remote options, depending on location. The base pay varies by state, and comprehensive benefits are included. You'll report to the Case Management Manager and must reside in one of the specified states. Requirements

• High school diploma or GED
• CHW certification required from approved Community Health Certification Program, or two years of equivalent work experience
• 1+ years experience care coordination and navigation

Responsibilities

• Assist members in following an outcome-based plan of care, based on the member's input and assessment results and identify/address current needs of the member, using outlined escalation pathways when needed
• Outreach identified members to identify and help solve for barriers that may affect their care by phone outreach, secure messaging and mail
• Monitor care and services provided to members to ensure that quality care is being delivered in the most appropriate setting
• Ensure the "Plan of Care" moves with the member and update the care plan as the member's health status changes
• Assist members by providing resources, education and information promptly
• Compliance with all applicable laws and regulations
• Other duties as assigned

Preferred Qualifications

• Bilingual [English and Spanish or Creole]
• AA or Bachelor's degree in social science or health related field
• Behavioral Health Experience
