Summary
Join Rapid Care USA as a Core Measures Specialist and contribute to our quality improvement initiatives. This role involves accurate data abstraction and compliance with Get With The Guidelinesยฎ (GWTG) Stroke and Heart Failure programs. You will play a key role in advancing patient care by ensuring adherence to evidence-based guidelines. Responsibilities include data abstraction and reporting, collaborating on quality improvement projects, providing education and training, and ensuring compliance with regulatory requirements. The ideal candidate will possess strong analytical and communication skills and experience in clinical data abstraction or quality improvement. Remote work and flexible scheduling are offered.
Requirements
- Possess proficiency in electronic health record systems (e.g., EPIC, Cerner) and clinical data systems
- Possess strong analytical skills with the ability to interpret and report complex data accurately
- Possess excellent interpersonal and communication skills for engaging with multidisciplinary teams
- Be highly organized and detail-oriented, with the ability to manage multiple projects effectively
- Have a minimum of 2 years of experience in clinical data abstraction or quality improvement
Responsibilities
- Perform detailed abstraction of clinical data from patient records for GWTG Stroke and Heart Failure registries
- Ensure the timely and accurate submission of data to meet national and accreditation standards
- Identify and resolve documentation in patient records and communicate the findings back to the client
- Collaborate with multidisciplinary teams to identify and address gaps in documentation and care processes
- Assist in the development and implementation of quality improvement projects focused on stroke and heart failure care
- Generate reports and dashboards to communicate performance data to stakeholders and leadership
- Provide education to clinical staff on core measure requirements and guidelines for stroke and heart failure management
- Conduct feedback sessions with staff to address performance gaps and improve compliance
- Work closely with quality, nursing, and clinical teams to ensure adherence to Joint Commission, CMS, and other regulatory requirements
- Participate in meetings and team calls related to stroke and heart failure care improvement
Preferred Qualifications
- Have experience in submitting data to stroke registries or quality improvement programs
- Have knowledge of healthcare regulations and compliance standards (e.g., HIPAA, Joint Commission)
- Possess proficiency in data analytics and reporting tools, such as Excel or Tableau
- Have RN, RHIA, RHIT, or other relevant healthcare certification
- Have certification in Stroke (SCRN) or Heart Failure (CHFN)
- Have hands-on experience with GWTG Stroke and/or Heart Failure registries
