Responsibilities

• Navigate patients through difficult and complex health concerns
• Be an empathetic listening ear and action-oriented guide for patients
• Build strong, trusting relationships with Medicare patients
• Identify and prioritize Medicare patients’ needs
• Assist patients to maintain a streamlined care continuum
• Develop patient care plans to holistically address social determinants of health
