Hospice Triage LPN

Join IntellaTriage's growing team as a Hospice Triage Nurse and enjoy the flexibility of working from home! Provide compassionate after-hours care to hospice and home health patients using cutting-edge technology. This role requires experience in end-of-life care and a fast-paced environment. You must possess a compact LPN license or be willing to obtain one, have high-speed internet, and be tech-savvy. Three weeks of paid remote training are provided, along with a company-provided laptop and headset. Requirements

• Have or be willing to obtain a Compact LPN license
• Live in/work from a Compact US state
• Have experience with end-of-life care
• Have high-speed internet
• Be tech-savvy, enjoy a fast-paced environment, and have keyboard competence
• Be willing to work 1 weekday shift 3:30 p.m.-12 a.m. CST and rotating Sat & Sun 7:30 p.m.-4 p.m. CST
• Have experience in hospice, palliative, end-of-life care and in fast-paced environments: ED, critical care, surgical services, etc
• Use your own high-speed internet
• Have a quiet space, away from noise and distraction, while you work (privacy/HIPAA compliant space is required)
• Be able to follow instructions, read directions, and be confident using technology
• Be comfortable accessing multiple technology applications to document during calls
• Have the ability and comfort with typing in a fast-paced environment
• Have fluency in English
• Physically reside in the U.S. and be legally eligible to work for any employer
• Be able to complete three weeks of remote paid training that is conducted during days and evenings
• Be available to work some Holidays as required
• Remain in good standing and ensure your home state license remains active
• Hospice, palliative, end-of-life care experience is required
• Experience in a fast-paced environment: ED, surgical services, or critical care, etc

Responsibilities

• Provide after-hours nurse triage for hospice and home health providers
• Manage after-hours care delivery
• Become an extension of our clients’ care team
• Access EMRs for charting
• Utilize internal applications to perform job functions
• Follow instructions and read directions
• Complete a minimum of 30 minutes paid shift prep prior to taking calls
• Spend 30 minutes to 2 hours at the end of your shift completing remaining charting before clocking out
• Handle stress and multitask when receiving calls (minimum of 5 calls per hour on weekdays, and up to 8 per hour on weekends)
• Communicate empathically with patients while adhering to protocols
• Maintain CEU’s as designated by the states in which you are answering calls
• Attend any in-services and additional training on an as-needed basis
• Pass background check and nurse licensing check
