Summary

Join Vesta Healthcare, a specialized medical group focused on aging adults with long-term home care needs, as a Nurse Practitioner. This role involves providing remote care on Saturday and Sunday nights (8 pm-8 am ET), handling an average of 5 calls per night. Responsibilities include call triage, care coordination, treatment recommendations, and clinical assessments. The ideal candidate will be a certified and licensed Nurse Practitioner in New York, with preferred licensure in Massachusetts and/or Pennsylvania, possessing relevant experience and strong clinical skills. Compensation includes an hourly rate plus a bonus, and the position offers the flexibility of remote work.

Requirements

  • Certified and licensed as a Nurse Practitioner in good standing in the state of New York
  • Master’s or doctoral degree from an accredited institution for nurse practitioners
  • Medicare participation and ability to have the company bill for services on your behalf
  • Certification from ANCC (or equivalent) as an Adult, Family, Geriatric, and/or Acute Nurse practitioner
  • 1+ years of Nurse Practitioner Experience
  • 1+ years of telephonic triage or equivalent experience
  • 2+ years of clinical experience working with complex adult populations
  • Ability to practice independently with little clinical support
  • Comfort using technology like Google Suite, multiple EMRs, Slack
  • The ability to work remotely and has a private area with a computer in their home/workspace

Responsibilities

  • Work every Saturday and Sunday overnight (8pm ET - 8am ET)
  • Triage inbound calls from our members and assess actions to be taken which could include prescribing uncontrolled substances when indicated
  • Conduct care coordination and recommend/identify cost effective research based treatment and intervention
  • Utilize strong clinical skills in physical assessment and chronic disease management for at risk adults and apply member specific Care Management and individualized care planning
  • Serve as a consulting resource on care management practice as needed
  • Attend meetings, training sessions as needed
  • Possess a strong knowledge of clinical procedures, standards and quality control checks
  • Possess a strong knowledge of medical conditions, interventions and treatment
  • Provide members, caregivers and facility education
  • Monitor the quality of member’s care and updates plan of care

Preferred Qualifications

  • Certified and licensed as a Nurse Practitioner in good standing in the state of Massachusetts and/or Pennsylvania
  • 1+ years of Nurse Practitioner Experience, qualified for independent practice in your licensed jurisdiction
  • Experience working in home care and/or family medicine, geriatrics
  • Experience working within a clinical team environment
  • Strong organizational skills, including the ability to prioritize

Benefits

Pay range is $35/hour plus a bonus based on services performed on each call
