Summary

The job is for a Remote Patient Monitoring (RPM) Care Coach position. Requirements

  • Possesses a Nursing (LPN, LVN, RN)
  • Knowledgeable about the basics of chronic health conditions related to the devices and the operation of those devices
  • Proficient with technology
  • Comfortable with navigating computer platforms and familiar with smart phone functionality
  • Comfortable with telephone outreach and engagement
  • Experienced with client interaction
  • Excellent communicator (written and verbal)
  • Highly organized and efficient
  • Detail-oriented and good at multi-tasking
  • Team Player
  • Exhibits a strong sense of ambition, self-motivation and self-discipline

Responsibilities

  • Serve as the point of contact for patients enrolled in the RPM365 platform by their health care professional
  • Help patients connect to the RPM 365 app and set up their devices
  • Engage regularly with all assigned patients to encourage consistency with the treatment plan and ensure they remain within the data range set by their provider
  • Schedule regular monthly or weekly check-ins with the patients
  • Monitor patients' data
  • Answer patient's alerts within M-F (9-5)
  • Adhere to the escalation protocols
  • Educate patients through shared information (articles, news, etc.)
  • Attend virtual meetings with RPM Healthcare's team for information updates

Preferred Qualifications

  • Tele-health experience
  • Patient monitoring experience
  • Patient Education experience
  • Additional language is a plus

Benefits

  • The pay is $22/hour – Not negotiable
  • After 2 months probational period and successful passing of orientation, you will become a W2 employee salary of $45,750 + bonuses with full benefit package (2 weeks PTO, 1 week sick, 8 holidays, health insurance and more)
  • Full time between 9:00 am and 5:30 PM Mon through Friday
