Summary

Join RPM Healthcare as an RPM 365 Care Coach and engage patients, track and analyze their data, provide education, and encourage adherence to their care plans. You will serve as the primary contact for patients on the RPM365 platform, assisting with device setup and app connectivity. Requirements

• Active nursing license in California (having a compact/multistate is ideal)
• Knowledge of chronic conditions and related monitoring devices
• Proficiency with technology, computer platforms, and smartphones
• Strong verbal/written communication skills
• Comfortable with patient outreach and engagement
• Highly organized, detail-oriented, and team-focused

Responsibilities

• Serve as the primary contact for patients on the RPM365 platform
• Assist with device setup and app connectivity
• Engage with patients to ensure adherence to care plans
• Schedule monthly check-ins and monitor patient data
• Educate patients through shared resources
• Notify healthcare providers of critical alerts
• Attend virtual team meetings for updates

Preferred Qualifications

• Experience in telehealth, patient monitoring, education, or customer service
• Additional language skills are a plus
