Responsibilities

• Document any dictated patient history, examination, labs, and medications
• Assist the physician with documentation of the patient’s medical record
• Transcribe the history and examination as given by the patient and the physician
• Transcribe the impression and plan, results of tests, prescriptions, and orders under the direction of the physician
• Transcribe any consultations or discussions with family members
• List all appropriate diagnoses and symptoms as well as follow-up instructions as directed by the physician
• Document medical visits and procedures as they are being performed by the physician accurately and thoroughly
• Perform clerical tasks as needed by the physician including faxing, phone calls, and referral letters
