Requirements
- Possess experience in a fast-paced medical environment
- Understand the principles of integrated care
- Be comfortable delivering brief, goal-oriented interventions
- Demonstrate strong communication, flexibility, and a proactive, collaborative approach
- Hold a Masterβs degree in Social Work, Psychology, Counseling, or a closely related field
- Possess current licensure as a Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or Licensed Marriage & Family Therapist (LMFT)
- Possess strong clinical assessment and intervention skills with a focus on brief, solution-focused therapy
- Demonstrate the ability to work collaboratively within a multidisciplinary team and communicate effectively with primary care providers
- Be comfortable with using telehealth technology for remote patient care
- Be flexible and adaptable to changing work environments and patient populations
- Demonstrate a commitment to providing culturally competent and trauma-informed care
- Attend work regularly and on time, as scheduled
Responsibilities
- Collaborate with medical providers and support staff to address patients' behavioral health needs within the primary care setting
- Provide brief, evidence-based interventions, psychoeducation, care coordination, and consultation to support patients experiencing a wide range of behavioral, emotional, and physical health concerns
