Requirements

  • Possess experience in a fast-paced medical environment
  • Understand the principles of integrated care
  • Be comfortable delivering brief, goal-oriented interventions
  • Demonstrate strong communication, flexibility, and a proactive, collaborative approach
  • Hold a Master’s degree in Social Work, Psychology, Counseling, or a closely related field
  • Possess current licensure as a Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or Licensed Marriage & Family Therapist (LMFT)
  • Possess strong clinical assessment and intervention skills with a focus on brief, solution-focused therapy
  • Demonstrate the ability to work collaboratively within a multidisciplinary team and communicate effectively with primary care providers
  • Be comfortable with using telehealth technology for remote patient care
  • Be flexible and adaptable to changing work environments and patient populations
  • Demonstrate a commitment to providing culturally competent and trauma-informed care
  • Attend work regularly and on time, as scheduled

Responsibilities

  • Collaborate with medical providers and support staff to address patients' behavioral health needs within the primary care setting
  • Provide brief, evidence-based interventions, psychoeducation, care coordination, and consultation to support patients experiencing a wide range of behavioral, emotional, and physical health concerns
