Summary

Join Carda Health, a company reimagining cardiac and pulmonary rehabilitation through a home-based telehealth program. As a Registered Nurse or Nurse Practitioner, you will be the initial point of contact for patients, conducting face-to-face visits and enrolling them in our program. You will provide high-quality clinical care, perform health histories, diagnose, and make referrals. This role requires expertise in cardiac and pulmonary pathophysiology, experience with telehealth, and licensure in multiple states. The position offers the flexibility of working remotely from anywhere in the United States.

Requirements

Responsibilities

  • Provide high-quality clinical care utilizing best practices set forth by Carda’s Clinical team
  • Be the first face-to-face touch point for many of the patients joining our program
  • Perform health histories, evaluate patient records, diagnose, and refer to our programs when appropriate
  • Provide lifestyle recommendations to patients related to specific goals
  • Input notes and all other relevant information into our EMR
  • Communicate and partner with other members of the patients’ care team to properly coordinate care and oversee patient care plans on an ongoing basis

Preferred Qualifications

  • Board Certified Nurse Practitioner
  • Licensed in the following states or least 25 states (ideally all 50, but the more the better): California, Texas, Florida, New York, Pennsylvania, Illinois, Ohio, Georgia, North Carolina, Michigan, Virginia, Washington, Arizona, Tennessee, Indiana, Missouri, Wisconsin, Maryland, Massachusetts, Colorado, Oregon, Minnesota, South Carolina, Nevada, Utah
  • Expertise in the management of individuals with cardiac & pulmonary pathophysiology
  • Cardiopulmonary training in basic life support or advanced cardiac life support
  • Experience prescribing and reviewing medication for cardiac and pulmonary patients
  • Passion for delivering high quality care to patients
  • Experience with care in a virtual setting and a desire to learn and influence new technologies
  • Highly organized with the ability to create, adhere to, and scale repeatable processes
