Requirements
- Minimum 5 yearsβ experience with a pediatric population required, Primary Care treating complex care, Pediatric Complex Care, or PICU
- You have completed an accredited program for Nurse Practitioner (NP) or Physician Assistant (PA-C) and hold an unrestricted NP or PA state license in Texas, New York, and Florida, including board certification
Responsibilities
- Manage a panel of complex pediatric patients, providing comprehensive oversight of their care plans and coordinating care across the healthcare system using a virtual platform
- Collaborate with pediatric RNs and other care team members, offering guidance and resources to support complex pediatric patients
- Educate and empower parents by providing tailored education and reinforcement of care plans to ensure comprehensive support within the home environment
- Leverage virtual platforms to conduct patient visits, ensuring care is accessible, timely, and patient-centered
- Partner with a multidisciplinary team including nurses, physicians, behavioral health therapists, dietitians, pharmacists, and mobile clinicians to deliver coordinated and optimized care
- Support new market launches by serving as a foundational anchor team during go-live phases
- Step in as needed to provide additional support to existing markets, ensuring continuity and high standards of care
Preferred Qualifications
- Medicaid enrollment in New York and/or Texas is highly preferred
- Willingness to become cross licensed in new states as requested based on business needs
