Join Jefferson Health at Home by BAYADA as a Weekend Home Health Physical Therapist to provide compassionate care to patients in Philadelphia, PA. As a pay per point position, you'll use your clinical skills and heart to treat patients in the comfort of their own home.

Requirements

• Active and current license to practice as a professional Physical Therapist (PT) in the state of PA
• Experience with OASIS strongly preferred
• Ability to travel to cases as assigned
• A calm and reassuring presence, active listening skills, excellent assessment skills and strong written and verbal communication
• The ability to deliver patient care as you would want those you love to be treated, with compassion, excellence, and reliability

Responsibilities

• See a client through their care needs – from hospital discharge to living a safe home life with comfort, independence, and dignity
• Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments
• Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions
• Continually assess and revise the Physical Therapy - PT care plan, and participate with nursing in the multidisciplinary care plan, as appropriate
• Implement integumentary protective interventions to prevent wound formation and promote wound repair utilizing evidence-based dressings and topical agents
