Requirements

• Must be a Licensed Clinical Social Worker (LCSW) in one or more of the following states: Alabama, Alaska, Arkansas, California, Colorado, Connecticut, Florida, Hawaii, Idaho, Indiana, Louisiana, Michigan, Minnesota, Missouri, Nebraska, Nevada, New Jersey, New Mexico, New York, North Carolina, Oklahoma, Oregon, Rhode Island, South Carolina, Tennessee, Virginia, Washington and Wisconsin
• Oncology experience required
• Certified Patient Navigator or experienced with patient navigation preferred
• Proficient in English and Spanish is a must

Responsibilities

• Conduct virtual comprehensive mental health evaluations/assessments to identify and address patients' emotional and social issues related to their cancer diagnosis
• Develop and implement treatment plans focusing on behavior modification, anxiety reduction, and stress management to support patients through their cancer journey
• Offer ongoing therapeutic support and case management, including cancer navigation services, to assist patients in managing their care effectively
• Accurately prepare and maintain treatment and progress reports, and ensure documentation meets Private Insurance and Medicare standards
• Collaborate with other agencies and healthcare professionals to coordinate comprehensive care and support for patients
• Provide timely and effective responses to crisis situations, including suicide risk assessments and the development of appropriate intervention plans and referrals
• Guide patients in accessing community resources, acting as an advocate and liaison for patients and their families
• Stay informed about local and regional cancer resources to enhance patient support and care
