Join Form Health as a virtual obesity medicine clinic delivering multi-disciplinary evidence-based obesity treatment through telemedicine. We are seeking obesity medicine physicians to provide longitudinal telehealth care for patients with obesity.

Requirements

• Have an MD or DO degree
• Be board certified and diplomates of the American Board of Obesity Medicine (ABOM)
• Be licensed to practice and prescribe legally
• Be credentialed with insurance payers in one or more of the following states: CA, WA, IL, KS, TX, MA, NY, NJ
• Have excellent interpersonal skills for patient and team interactions
• Be committed to the highest quality of medical care and an evidence-based approach to obesity management
• Have prior experience conducting telemedicine visits
• Have a great 'web-side' manner
• Have prior experience working collaboratively with advanced practice professionals and registered dietitians

Responsibilities

• Provide longitudinal telehealth care for patients with obesity
• Collaborate with advanced practice professionals and registered dietitians towards exceptional team-based care
