Summary

Join Solace as an RN Healthcare Advocate and guide Medicare patients through complex health concerns, helping them achieve their health goals while addressing Social Determinants of Health (SDOH). This 1099 role offers part-time or full-time remote work. You will build strong patient relationships, identify and prioritize their needs, and develop comprehensive care plans. You'll leverage your clinical skills and organizational abilities to manage multiple tasks and learn new systems. Solace is a Series B startup transforming healthcare by connecting patients with expert advocates. This role requires an RN license and a deep understanding of SDOH.

Requirements

  • RN license in good standing
  • Deep understanding of Social Determinants of Health and experience working with diverse patient populations
  • Endless empathy for people, and a strong ability to fight for those who cannot
  • Strong clinical skills paired with exceptional organizational abilities. You can balance multiple tasks and work under pressure without sacrificing clarity in your communications and documentation
  • Pride in your technical savvy; you can quickly and fluently learn new systems and software
  • An extreme bias toward action and execution
  • A willingness to provide fearless feedback. You care about forging a system that empowers better patients outcomes, and are not shy about sharing your thoughts
  • Applicants must be based in the United States

Responsibilities

  • Learn the Solace systems, tools, technology, partners, and expectations, while also providing your unique expertise in every interaction
  • Build strong, trusting relationships with Medicare patients, where listening and empathy are the foundation for every interaction
  • Be able to identify and prioritize Medicare patients’ needs and assist them to maintain a streamlined care continuum
  • Develop comprehensive patient care plans that holistically address social determinants of health, i.e. food resources, transportation access, and support at home
  • Build the systems of the future in working with Medicare patients
