Summary

The Clinical Social Worker will assist the primary care manager in managing the overall health of their enrolled population, participate in group medical appointments, develop and implement clinical pathway programs, document care and recommendations, consult with various personnel as required, and attend meetings. A Master of Social Work (M.S.W.) degree and a license to practice psychology are required. The position also requires certification in either the American Heart Association Basic Life Support (BLS) Course C or the American Red Cross CPR/BLS course.

Requirements

  • At least one (1) year of experience with assessing and providing evidence-based individual counseling to patients within a mental health care facility within the past five (5) years
  • Shall be skillful and tactful in communicating with people who may be physically or mentally ill, uncooperative, fearful, emotionally distraught, and occasionally dangerous
  • Shall possess organizational, problem-solving, and communication skills to articulate medical requirements to patients, families/care givers, medical and non-medical staff in a professional and courteous way
  • Master of Social Work (M.S.W.) degree
  • License to practice psychology in any of the fifty United States, the District of Columbia, the Commonwealth of Puerto Rico, Guam, or the U.S. Virgin Islands
  • Certification in either the American Heart Association Basic Life Support (BLS) Course C or the American Red Cross CPR/BLS course

Responsibilities

  • Assist the primary care manager (PCM) in managing the overall health of their enrolled population
  • Participate in group medical appointments, including Shared Medical Appointments (SMA) and Drop-in Group Medical Appointments (DIGMA)
  • Provide concise documentation of care and recommendations in the patient’s electronic medical record within the allowable timeframe defined by the MTF standard after the patient is seen
  • Consult with medical personnel, legal authorities, and military commanders as required
  • Attend and participate in meetings during normal duty hours and professional staff conferences and other appropriate professional activities
