Requirements

  • Medical degree (MD or DO) from an accredited institution
  • Board certification in Internal Medicine, Family Medicine, Endocrinology, Cardiology, Obesity Medicine, or a related specialty
  • Active medical license in the state(s) where telehealth services will be provided
  • Commitment to maintaining high standards of professionalism, ethics, and patient safety in a virtual care setting

Responsibilities

  • Provide a combination of synchronous video consultations and asynchronous care, including direct messaging, for patients diagnosed with cardiometabolic conditions such as diabetes, hypertension, hyperlipidemia, and obesity
  • Perform comprehensive medical assessments, review patient medical histories, and order appropriate diagnostic tests as needed
  • Develop individualized treatment plans tailored to each patient's unique needs, incorporating evidence-based guidelines and best practices
  • Prescribe medications, lifestyle modifications, and other therapeutic interventions to optimize patient health outcomes
  • Educate patients about their conditions, treatment options, medication adherence, and preventive measures to minimize disease progression and complications
  • Monitor patient progress through regular follow-up appointments, remote monitoring devices, and virtual check-ins
  • Review cases with our multidisciplinary team and approve recommendations from clinical pharmacists and registered dietitians
  • Collaborate with our team of dietitians and other healthcare professionals to coordinate whole-body care and support patient wellness goals
  • Utilize telehealth technology and electronic health records (EHR) systems efficiently to document patient encounters, update medical records, and communicate with the care team
  • Stay abreast of the latest developments in cardiometabolic medicine, telehealth practices, and digital health technologies through continuing medical education (CME) activities and professional networking
  • Adhere to regulatory requirements, privacy laws, and company policies to ensure compliance with telemedicine standards and safeguard patient confidentiality

Preferred Qualifications

  • Experience practicing telemedicine or providing remote patient care
  • 2+ years of outpatient clinical experience post-residency or fellowship
  • Strong clinical knowledge and expertise in managing cardiometabolic conditions, including familiarity with current treatment guidelines and therapeutic modalities
  • Excellent communication skills, both verbal and written, with the ability to explain complex medical concepts in a clear and accessible manner
  • Empathetic bedside manner and a patient-centered approach to care
  • Proficiency in using telehealth platforms, video conferencing software, and electronic health records (EHR) systems
