Virtual Nurse Practitioner

Summary

Join our team as a Virtual Nurse Practitioner and be part of transforming healthcare by making quality care more affordable, accessible, and enjoyable. As a key member of our One Medical at Home high-risk care team and High Risk Care Management virtual team, you will care for geriatric, frail, and clinically complex patients through a team partnership with the High Risk Care Medical Director and high risk teams, as well as the in-office Primary Care Providers.

Requirements

• Completed an accredited NP program with a national certification (ANCC, AANP) in Family, Geriatrics, or Adult
• 5+ years of experience as a Nurse Practitioner with at least 2 years of home-based, in-office or virtual complex care, Geriatric Primary Care, geriatrics or palliative care experience
• Bilingual in Spanish and English highly preferred
• Currently licensed in Washington and able/willing to obtain licensure in the state of Arizona and additional States as requested
• Must be based in PST time zone
• Must be willing to flex between PT/MT with advanced notice
• Experience with home-based care preferred, specifically in the Seattle metropolitan area
• Experience with value-based care principles and risk adjustment
• The ability to build successful relationships with team members and communicate effectively both 1-on-1 and in groups
• The ability to work with care coordinators to direct care in the home through tele-assist capabilities
• Strong technology skills and ability to use telehealth platforms
• Can thrive in a fast-growing, mission-driven organization focused on using data to improve patient outcomes

Responsibilities

• Complete thorough initial visits with patients, diving deep into medical, social, and behavioral context
• Complete ongoing follow-up visits with patients, focusing on geriatrics and chronic condition management
• Complete post-discharge visits on recently admitted patients, including comprehensive medication reconciliation, Specialist coordination, and chronic condition management
• Complete engagement and outreach visits on patients who have not been seen by their primary care provider this year or who have chronic condition opportunities
• Work directly with the patient and through the multidisciplinary care team to impact ongoing care with particular emphasis on preventable hospitalizations and ensuring care aligned with patient goals
• Collaborate and coordinate with the patient's primary high risk care team and primary care team and specialists
• Participate actively in team huddles to review care plans and assess patient goals of care and care plans
• With support of the team, respond to patient and patient's family telephone calls
• Participate in the market after-hours call pool
