Virtual Nurse Practitioner

Summary

Join One Medical, a primary care solution challenging the industry status quo, as a full-time Virtual Nurse Practitioner to join our One Medical at Home, high-risk care team and High Risk Care Management virtual team. The Virtual Nurse Practitioner will care for geriatric, frail, and clinically complex patients through a team partnership with the High Risk Care Medical Director and high risk teams, as well as the in office Primary Care Providers.

Requirements

• Completed an accredited NP program with a national certification (ANCC, AANP) in Family, Geriatrics, or Adult
• 5+ years of experience as a Nurse Practitioner with at least 2 years of home based, in office or virtual complex care, Geriatric Primary Care, geriatrics or palliative care experience
• Bilingual in Spanish and English highly preferred
• Currently licensed in Arizona and able/willing to obtain licensure in the state of Washington and additional States as requested
• Must be based in PST or MST time zone
• Must be willing to flex between PT/MT with advanced notice

Responsibilities

• Complete thorough initial visits with patients, diving deep into medical, social and behavioral context
• Complete ongoing follow up visits with patients, focusing on geriatrics and chronic condition management
• Complete post-discharge visits on recently admitted patients, including comprehensive medication reconciliation, Specialist coordination, and chronic condition management
• Complete engagement and outreach visits on patients who have not been seen by their primary care provider this year or who have chronic condition opportunities
• Work directly with the patient and through the multidisciplinary care team to impact ongoing care with particular emphasis on preventable hospitalizations and ensuring care aligned with patient goals
