Virtual Support Nurse

Summary

Join Spectrum Health Care as a Virtual Support Nurse, providing remote assessments and triage for patients to enable them to receive care at home. This hybrid role combines in-office and remote work, allowing you to utilize your nursing skills to support patients and their families. You will be part of an integrated team, providing 24/7 clinical virtual support on a scheduled rotation. As the first point of clinical contact, you will assess patient needs, determine next steps, and coordinate in-person care when necessary. You will also assist with symptom management, navigation, and education. This position offers the opportunity to be part of a collaborative team and make a real difference in patients' lives.

Requirements

• Registered Nurse (RN) or Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario
• Ability to work in our Mississauga office
• Demonstrated ability to perform comprehensive clinical assessments virtually and make independent decisions

Responsibilities

• Provide remote assessment and virtually triage support for home care clients
• Assess patient needs and determine the appropriate next steps
• Coordinate the necessary follow-up support for direct clinical care when in-person care is required
• Support the client and care partners with symptom management, navigation and education
• Provide 24/7 clinical virtual support on a scheduled rotation

Preferred Qualifications

Minimum 3 years of clinical experience in a community, medical surgical departments, emergency department, or triage setting
