Summary
Join the Masonic Homes of California as a MSOS Care Manager and coordinate care services for senior fraternity members needing assistance with aging and financial issues. This remote position, based in North LA County, involves extensive travel (50-75%) throughout Northern LA, Ventura, Kern, and Fresno Counties. You will perform client assessments, develop care plans, and maintain client communication. The role requires advocating for clients, managing budgets, and maintaining accurate online database information. Occasional weekend and evening work may be required, along with occasional overnight and out-of-state travel. Requirements
- BA is required +three or more years of work experience in related field
- Knowledge of the aging process, health, and social service system
- Experience with obtaining benefits through Medicare, Medi-Cal, and other insurance
- Must be able to exercise independent judgment, demonstrate good assessment skills and relate effectively to the elderly, professional associates, and clients’ families
- Must possess and maintain a valid California driver’s license without restrictions
Responsibilities
- Performs assessment to evaluate physical, functional, financial, emotional, and environmental status of clients
- Develops, monitors, and maintains individual care plans for clients to provide appropriate and adequate resources to support their health, safety, and changing needs
- Understands basic concepts of confidential of client information and requirements for permissible disclosure
- Develops and maintains personal connections with clients as well as client families, peers, Masonic Homes of California departments, Masonic Lodges, vendors and external agencies involved in clients care plans
- Maintains a regular visit schedule with clients
- Informs clients and appropriate family members of all public benefits to which they may be entitled and provides guidance in obtaining these benefits
- Maintains ongoing open communication among all care providers, lodges, and family within privacy/confidentiality requirements
- When necessary, acts as an advocate for client to ensure fair and equitable outcomes
- Possesses well-versed knowledge about all Masonic Homes of California programs and services
- Develops, monitors, and adheres to client budget and requests for assistance for extraordinary occurrences based on current information and according to MOS (Masonic Outreach Services) guidelines. Responsible for reporting any major changes in status to management in a timely manner
- Receives and reviews reports, and correspondence as needed
- Performs timely updating of and maintains accurate information on the online database, demonstrating a thorough knowledge of the system
- Performs related duties as assigned
- Travel throughout assigned regions and to MHs campuses
- Occasional overnight and out-of-state travel is also required
- No on-call duties required
- Participate in lodge presentations regarding program services, on occasion
Preferred Qualifications
Master's Degree preferred + two or more years of work experience in related field
