Summary

Requirements

  • Experience: 2-3 years in patient navigation, healthcare coordination, or medical records management
  • Healthcare Knowledge: Familiarity with medical terminology, patient care processes, and documentation best practices
  • Attention to Detail: Strong accuracy in medical documentation and formatting
  • Critical Thinking: Ability to recognize when to escalate concerns to physicians
  • Adaptability: Comfortable with evolving processes and ambiguous situations

Responsibilities

  • Conduct initial patient assessments, recording symptoms and concerns
  • Accurately transcribe physician notes, diagnoses, and treatment plans
  • Maintain up-to-date and compliant medical records
  • Document key patient details into Haven Headache’s care plan system
  • Work with physicians to ensure accurate and thorough patient documentation
  • Assist in creating and updating personalized care plans
  • Follow up with patients post-appointment to track treatment adherence
  • Educate patients on medications, lifestyle changes, and preventive care
  • Gather patient feedback to enhance care and support services
  • Collaborate with physicians, nurses, and admin staff for seamless care
  • Handle insurance documentation, prior authorizations, and billing inquiries
  • Ensure compliance with healthcare regulations and quality standards
  • Keep patient records updated, especially after treatment changes
  • Ensure timely responses to patient inquiries and operational efficiency
  • Support insurance and billing teams in assisting patients with claims
  • Learn the company's care plan system and documentation process
  • Start managing and updating care plans, ensuring accuracy in dosages and prescriptions
  • Understand how the care plan system functions within Google Docs
  • Convert doctor’s notes into structured, well-organized care plans
  • Aim for a 30-minute turnaround time per care plan (some may take up to an hour)

Preferred Qualifications

  • Pharmacy Background: Preferred over nursing for stronger familiarity with medications
  • Electronic Health Records (EHR) Expertise: Experience managing patient records in EHR systems
  • Care Plan Documentation: Ability to create and maintain legally compliant care plans
  • Future Growth Potential: Interest in expanding into broader operational roles over time
