Requirements

• A Nurse Practitioner with relevant experience
• A Bachelor of Science in Nursing
• A compact unrestricted Registered Nurse (RN) license in a compact state
• A minimum of 3 years of experience in adult oncology

Responsibilities

• Conduct high-touch case management, overseeing approximately 75-80% of clinical aspects for Case Management RNs
• Lead weekly case conferences to collaboratively review patient scenarios and refine care plans with the nursing team
• Provide clinical input and serve as an escalation point for patient care within designated pods
• Review patient information to determine appropriate levels of care upon entry to Thyme Care
• Engage in chart reviews, care plan creation, and clinical guidance for oncology-trained nurses
• Participate in urgent care shifts as needed, typically limited to 1-2 weekly shifts, without weekend or after-hours responsibilities
• Collaborate with the daytime APP leader and three Medical Directors for support and escalation needs
• Prioritize patient needs, focusing on triage to prevent hospital admissions and addressing social determinants of health
