Join Fella & Delilah as their first Fractional Medical Director in Canada, a part-time, remote contractor role (5-10 hours/month initially), with potential for growth. You will play a crucial role in defining safe, scalable, and compliant healthcare for Canadian patients. Responsibilities include serving as the Responsible Physician, reviewing and approving clinical protocols, providing oversight for complex cases, and ensuring compliance with Canadian regulations. You will collaborate with various teams and contribute to shaping the clinical model from the ground up. The role involves building systems and protocols, navigating interprovincial medical leadership rules, and advising on local prescribing policies as the company expands its services.
Requirements
- Licensed MD or DO, active, unrestricted license in at least 3 Canadian provinces (BC, ON, AB)
- Eligible to serve as Medical Director / Responsible Physician per College regulations
- Comfortable providing oversight for asynchronous and low-touch care models , especially in primary care or obesity medicine
- Experience with GLP-1s, TRT, or other chronic condition prescribing
- Willingness to create protocols, respond to edge cases , and help document our medical rationale for compliance
- Strong understanding of clinical ethics, medical liability , and regulatory frameworks in Canada
- Strong documentation and process-thinking skills — you like building systems and protocols, not just practicing medicine
Responsibilities
- Serve as the Responsible Physician for reviewing and signing off on Fella’s protocols, patient safety practices, and medical SOPs in Canada
- Review and approve clinical protocols for GLP-1 prescribing and other medical care in alignment with provincial guidelines and College standards
- Provide oversight and escalation support for complex clinical cases or edge scenarios, including adverse events and prescription eligibility edge cases
- Work cross-functionally with the Ops, Product, and Regulatory teams to ensure compliance with medical legislation and licensing frameworks in each province
- Build and document defensible medical justifications for our asynchronous and low-touch care model (e.g. rationale for async, patient triage logic, low visit frequency)
- Help us navigate interprovincial medical leadership rules , including supervision requirements and medical delegation if needed
- Oversee or delegate oversight of additional NPs and physicians as we scale province by province
- Audit and continuously refine medical decision-making in our workflows (e.g. prescribing logic, escalation pathways, lab result thresholds)
- Advise on local prescribing policies, controlled substance management , and pharmacy relationships as our care offering expands (e.g. longevity, testosterone, or future adjunct meds)
Preferred Qualifications
- Experience as a Medical Director or supervising physician in a virtual care or startup setting
- Familiarity with obesity medicine, endocrinology, or men’s health
- Interest in longevity medicine or familiarity with treatments like metformin, NAD+, or peptides
- Experience interacting with College regulatory bodies , insurance carriers, or pharmacists
