Health Coach

Summary

Join Omada Health as a Health Coach and guide members through online disease prevention and management programs. Leverage our online platform and data science to coach members individually and in online forums. Collaborate with the care team to ensure comprehensive care and contribute to best practices. This hourly position offers $23/hr for 35-45 hours/week with schedule flexibility within business hours. You'll support members with chronic conditions, helping them set and achieve health goals. Responsibilities

• Guide members through Omada’s online disease prevention and disease management programs
• Communicate one-on-one with members through our platform
• Facilitate conversation in online discussion forums
• Collaborate with other members of our care team to ensure you are providing comprehensive care
• Partner with other members of the Health Coaching and Operations teams to discover and drive digital health coaching best practices
• Provide ongoing support to members who are living with, or are clinically at risk for developing, a chronic condition such as type 2 diabetes or hypertension
• Collaborate with members to help address barriers, set goals, and make progress toward their health goals

Preferred Qualifications

• Have successful experience as a virtual health coach, community health worker, or health educator
• Have an active health coaching certification (e.g., NBHWC, Wellcoaches, Duke Integrative)
• Have an active Diabetes Prevention Program (DPP) Lifestyle Coach certification
• Have a personal connection to behavior change, chronic disease, or a health journey
• Be bilingual/bicultural
