Requirements

  • High school diploma or GED required
  • Must have a valid NYS drivers license and reliable vehicle
  • Up to 25% Travel within Syracuse, NY area required
  • Strong computer skills and ability to learn and use new technology
  • Empathy, compassion, and patience
  • Superior listening, verbal, and written communication skills
  • High level of professionalism and ability to work independently and with a team remotely
  • Strong problem-solving abilities with attention to detail
  • Ability to handle stressful situations appropriately

  • Must be able to work from home with a HIPAA compliant environment
  • Must be flexible to work some evenings, holidays and weekends

Responsibilities

  • Monitor all alerts
  • Answer, direct, and follow up on incoming calls
  • Review and maintain follow up tasks
  • Receive and perform virtual follow up calls and document all information obtained on the call in the appropriate charting systems
  • Support participants' physical and emotional wellness with a holistic health approach
  • Addresses social determinants of health by supporting participants through loneliness, anxiety, and depression
  • Communicate concerns, changes in condition and all medical complaints to the Registered Nurse
  • Troubleshoot issues with technology and escalate technical difficulties that cannot be resolved
  • Schedule, manage and complete participant home visits to deploy devices for new patients and provide in-home education and troubleshooting as needed
  • Other duties as assigned
