Summary
Join our team as an Epic Healthy Planet Analyst for a 4-month contract! You will design, implement, and optimize Epic's Healthy Planet module to enhance population health initiatives and value-based care. This role involves close collaboration with clinical, operational, and IT stakeholders to develop tools and analytics for proactive patient outreach and care coordination. Onsite work is required from June 2-13 (M-F), otherwise remote work is permitted. You will be responsible for building and maintaining Healthy Planet registries, dashboards, and supporting implementation of social determinants of health documentation. The position requires strong Epic application experience and a deep understanding of value-based care.
Requirements
- Possess 3 out of the following 4 Epic Certifications: Healthy Planet and Compass Rose Clinical Build, Healthy Planet and Compass Rose Analytics Build, Healthy Planet Value-Based Analytics Build, Regulatory Reporting: Quality Measures for Eligible Clinicians Badge
- Have 3+ years of experience with Epic applications, preferably in population health or clinical workflows
- Demonstrate a strong understanding of value-based care models, care management, and clinical quality improvement
Responsibilities
- Collaborate with clinical and operational leaders to assess population health needs and translate them into Healthy Planet workflows and reporting solutions
- Design, build, test, and maintain Healthy Planet registries, care gaps, scorecards, and dashboards within Epic
- Support the implementation of social determinants of health (SDOH) documentation, patient outreach tools, and clinical quality metrics
- Work with data analysts and reporting teams to ensure accurate data capture and reporting for value-based care and regulatory programs (e.g., HEDIS, CMS ACO programs)
- Troubleshoot application issues, perform root cause analysis, and resolve system-related problems
- Participate in Epic upgrade planning and testing, ensuring new functionality is integrated into existing workflows
- Develop and maintain documentation, training materials, and user support tools
- Provide ongoing support and training to end-users, clinical staff, and leadership
- Stay current with Epic enhancements, healthcare industry trends, and population health strategies
Preferred Qualifications
- Have experience with CMS ACO, HEDIS, and other quality reporting frameworks
- Possess knowledge of SDOH documentation and community-based care coordination
- Have a background in data analytics, reporting tools (e.g., SlicerDicer, Clarity, Caboodle), or SQL
- Have clinical or healthcare operations experience
