Join Hazel Health, a leading virtual provider of school-based healthcare, as a Licensed Pediatric Mental Health Intake Therapist (Virtual). This 1099 independent contractor position offers flexibility while making a significant impact on children's mental health. Conduct initial mental health intakes and assessments for K-12 students, collaborating with other therapists and Hazel's care management team. Provide culturally relevant counseling and treatment, and maintain relationships with school personnel and families. This role requires a minimum of 3 years of licensure as an LISW, LMFT, or LMHC in Iowa, plus an additional license in select states or eligibility for immediate application. Proficiency in technology and a commitment to flexible availability are essential. Note that this is a 1099 contractor position without benefits like health insurance or paid time off.

Requirements

  • Licensed without restrictions as a Licensed Independent Social Worker (LISW) , Licensed Marriage and Family Therapist (LMFT),or Licensed Mental Health Counselor (LMHC)
  • Licensed in Iowa for a minimum of 3 years
  • Additional license in AL, CA, CO, FL, GA, HI, MA, MD, MO, NV, TX, VA, or WA; or be eligible and able to begin a state license application process immediately upon signing your contract agreement
  • Willingness/ability to be enrolled by Hazel with insurance companies, including Medicaid and private payers
  • Possess a working knowledge of treatment strategies and interventions related to the mental health needs of children and their families
  • Demonstrates an ability to make accurate and timely clinical judgments or recommendations
  • Works in collaboration with other clinical team members across Hazel Health to meet the needs of children and families
  • Demonstrates experience and education in all areas of clinical practice
  • Highly proficient in computer and technology skills, including: the ability to use a laptop, quickly learn Hazelโ€™s clinical platform, navigate Google and Slack software applications, learn new software and programs and complete digital paperwork
  • Flexible Availability: You are expected to maintain a schedule that accommodates student needs, which may include availability during school hours or after-school hours, depending on district requirements. Current need is therapists that are available Monday-Friday after 4pm CST
  • Minimum Weekly Hours: While you have control over your schedule, there will be a minimum weekly time commitment based on caseload requirements, to be discussed during the onboarding process
  • Caseload Management: You are responsible for managing your own caseload, ensuring timely intakes, assessments, and follow-up sessions as needed
  • Communication: You must be responsive to communication from school personnel, families, and our care management team, and participate in occasional virtual meetings or trainings
  • Participate in 15 hours of paid training during initial onboarding and ongoing as needed to access internal systems and platforms
  • Credentialing: Hazel will credential you with insurance payors, the time spent is reimbursable time
  • Computer (additional monitor encouraged) either desktop or laptop, that has a webcam and microphone
  • Stable home internet connection with at least 20 mbps connection
  • Malpractice Insurance: You must obtain malpractice coverage at a minimum coverage of $3 million per claim and $3 million aggregate
  • HIPAA Compliant Workspace: You must be able to complete the functions of your job in a private, unshared and secure workspace
  • Mission-Alignment: You are inspired by providing equitable access to mental health care to students

Responsibilities

  • Conduct initial intakes and assessments with children in grades K-12 and their parent/guardian
  • Collaborate with other Hazel Mental Health Therapists to ensure children successfully transition into our services
  • May provide ongoing virtual counseling and solution-focused therapeutic interventions for children in grades K-12, as needed
  • Provide culturally relevant and age-appropriate counseling and treatment
  • Collaborate with Hazelโ€™s care management team to identify and support transition services
  • Maintain relationships and engage with school personnel, family members, and stakeholders
  • Obtain necessary state- or district-required credentialing, as requested by Hazelโ€™s credentialing department
