Obesity Medicine Physician

Summary

Join Form Health, a virtual obesity medicine clinic, as an obesity medicine physician providing telehealth care. You will collaborate with a team of advanced practice professionals and registered dietitians to deliver exceptional patient care remotely. Flexible schedules are available, and we offer both W2 employed and 1099 contractor positions. We are seeking board-certified physicians with experience in telemedicine and a commitment to evidence-based obesity management. Successful candidates will possess excellent interpersonal skills and ideally hold multiple state licenses. A competitive salary and comprehensive benefits package are offered.

Requirements

• MD or DO degree with board certification; must be an American Board of Obesity Medicine (ABOM) diplomate
• Be licensed to practice and prescribe legally and be credentialed with insurance payers in one or more of the following states: CA, WA, IL, KS, TX, MA, NY, NJ
• Have excellent interpersonal skills for patient and team interactions
• Be committed to the highest quality of medical care and an evidence-based approach to obesity management
• Have prior experience conducting telemedicine visits and have a great "web-side" manner
• Have prior experience working collaboratively with advanced practice professionals and registered dietitians

Responsibilities

• Provide longitudinal telehealth care for patients with obesity
• Collaborate with advanced practice professionals and registered dietitians towards exceptional team-based care

Preferred Qualifications

Be bilingual candidates with medical fluency in Spanish
