Summary

Requirements

• Bachelor's degree in exercise physiology is required
• Licensed as an Occupational Therapist in Pennsylvania
• 3+ years of experience as an Occupational Therapist, with a well-rounded background spanning acute care, rehabilitation, long-term residential care, home care, and outpatient settings
• Certified in Basic Life Support
• Maintenance of certification and continuing education credits
• Proficiency with telemedicine and/or remote care extension
• Reliable high-speed internet connection (at least 25 Mbps)

Responsibilities

• Provide care via telemedicine/remote care extension to patients suffering from advanced chronic lung disease
• Collaborate closely with other medical team members, often speaking multiple times per day with the supervising nurse practitioners and physicians
• Provide individual and group patient education on disease management, medication regimens, preventive care, smoking cessation, and many other topics
• Assess, triage, and escalate care for patients with worsening or acute symptoms
• Coordinate care with primary care providers, specialists, and external care agencies
• Advocate for patients who are experiencing major social stressors by activating community resources to improve access to medications, transportation, healthy nutrition, and safe housing
• Serve as the continuous point of contact for patients and families across the entire care spectrum
• Promote our program and answer questions from referring providers and practices as we become more established in your local community

Preferred Qualifications

Masters in occupational Therapy is preferred
