Summary
Join Synapticure, the largest specialty care clinic for neurodegenerative diseases, as an Outreach Specialist. You will be responsible for connecting with patients, engaging them regarding their needs, and transitioning them to a Care Coordinator. This role involves telephonic, written, and digital outreach, leveraging EMR data, and completing the intake process. You will be the patient's initial point of contact, providing support and education. The position is fully remote, offering a competitive salary and benefits package. Success in this role requires customer service or sales experience, strong interpersonal skills, a metrics-driven approach, and comfort with high-volume outbound calls.
Requirements
- Have 0-2 years of customer service or sales experience with a successful track record to share for that time period
- Have a passion for working within the community you are a part of, or have been a part of in the past
- Have dynamic interpersonal skills and a strong desire to improve the health of individuals and whole communities
- Have a metrics-driven approach to your work and are willing to do whatever it takes to meet quotas for the Outreach team
- Be comfortable with high-volume outbound calls as well as creative outreach
- Be an independent self-starter who responds well to change and shifting priorities
- Be excited about how technology can support your work and help drive the ongoing evaluation toward new, better, care
Responsibilities
- Engage eligible patients through telephonic, written, and digital outreach methods
- Leverage available data from EMR to learn more about potential patients and inform outreach processes
- Complete the intake and consent process for eligible patients, and transitioning them to a Care Coordinator upon full enrollment
- Engage and foster trust quickly with patients, acting as the face of Synapticure and the key point-of-contact to assist patients prior to full enrollment
- Connect authentically with patients and their families in a non-judgmental, respectful, and empathic manner and striving to meet their needs
- Work daily within EMR to collect data, organize information, track tasks, and communicate with staff, patients, and family members;Β Provide feedback to the product development team, and, over time, become part of a super-user group to assist in onboarding and supporting others
