Join Imagine Pediatrics, a tech-enabled pediatric group, as a Pediatrician providing 24/7 care for children with special healthcare needs from their homes. You will manage a panel of complex pediatric patients, overseeing care plans and coordinating with other healthcare professionals. Responsibilities include overseeing a multidisciplinary team, educating parents, conducting virtual and in-home assessments, providing gap care, and supporting administrative needs. This remote role requires an unrestricted MD or DO with an active New York state license, board certification, 5 years post-residency experience, and enrollment in New York Medicaid. Requirements
- Hold an unrestricted MD or DO with an active New York state license
- Board Certification in Pediatrics and/or Internal Medicine-Pediatrics
- Enrollment in New York Medicaid required
- 5 years post residency experience
- Willingness to obtain additional state licensure as requested
- Extensive experience managing children with medical complexity
Responsibilities
- Manage a panel of complex pediatric patients, comprehensively overseeing their care plans and coordinating with primary care providers, specialists, and other healthcare professionals to ensure seamless care
- Oversee a multidisciplinary team to provide wraparound support, working closely with paramedics to deliver in-home care (virtually) and address both medical and administrative, and social needs
- Educate and empower parents to understand and navigate their childβs medical care plan, reinforce treatment goals, navigate the healthcare system and ensure long-term health success
- Conduct comprehensive virtual enrollment visits for new patients, virtual acute and sick visits for current patients, and hybrid in-home virtual assessments for sick visits conducted by our Mobile Integrated Care team (paramedics), and post-hospital discharge follow-ups
- Provide gap care when patients experience a lapse in services, ensuring continuity and preventing setbacks in their treatment journey
- Support administrative medical needs, including coordinating prescriptions, referrals, and insurance-related processes to remove barriers to care
Preferred Qualifications
- Focus on quality of care, population health and/or virtual care experience a plus
- Experience with Medicaid and/or Community health preferred
- Value-Based Care experience a plus
- Bilingual Spanish a plus
