Requirements
- Be a graduate of an accredited School of Nursing
- Have a current unencumbered license to practice as a Registered Nurse
- Have a minimum of 1 year of hospice experience with a working knowledge of Title XX11, Medicare Hospice Conditions of Participation, and HIS
- Possess excellent written and oral communication skills
- Have the ability to handle stressful situations/deadlines
Responsibilities
- Review electronically submitted documentation and provide feedback to clinicians regarding deficiencies or Hospice Item Set (HIS) corrections to ensure the medical record reflects compliance with hospice eligibility guidelines
- Ensure visit utilization is supported by individual patient assessment
- Review additional orders for appropriateness and compliance
- Ensure clinical documentation accurately reflects recertification/discharge decisions. Prepare hospice CTI/recertification workflow prior to IDG meetings
- Review and approve Initial Hospice Plan of Care within HCHB
- Review HIS for inconsistencies and locks timely
- Collaborate with clinicians as necessary regarding documentation
- Partner with the Quality Director on a variety of quality initiatives and attend quarterly QAPI meetings
- Monitor performance improvement initiatives regarding patient outcomes
- Maintain working knowledge of hospice regulations, conditions of participation and HIS
- Review patient assessments to ensure appropriate and effective care is provided to each patient in compliance with agency policies, procedures, and regulatory requirements
- Communicate trends or patterns of inaccuracy identified to the Quality Director to assist in the development of the performance improvement and education plan
- Process and review clinical documentation
Preferred Qualifications
Possess strong computer and software skills with knowledge of Home Care Home Base
