Join Jasper Health's team as a Per Diem Licensed Clinical Social Worker (LCSW) to provide vital behavioral health and psychosocial support to cancer patients. Conduct mental health assessments, develop treatment plans, offer case management, and more.

Requirements

• Must be a Licensed Clinical Social Worker (LCSW) in one or more of the following states: Alabama, Alaska, Arkansas, California, Colorado, Connecticut, Hawaii, Idaho, Indiana, Louisiana, Michigan, Minnesota, Missouri, Nebraska, Nevada, New Jersey, New Mexico, New York, North Carolina, Oklahoma, Oregon, Rhode Island, South Carolina, Tennessee, Virginia, Washington and Wisconsin
• Oncology experience required
• Certified Patient Navigator or experienced with patient navigation preferred

Responsibilities

• Conduct virtual comprehensive mental health evaluations/assessments to identify and address patients' emotional and social issues related to their cancer diagnosis
• Develop and implement treatment plans focusing on behavior modification, anxiety reduction, and stress management to support patients through their cancer journey
• Offer ongoing therapeutic support and case management, including cancer navigation services, to assist patients in managing their care effectively
• Accurately prepare and maintain treatment and progress reports, and ensure documentation meets Private Insurance and Medicare standards
• Collaborate with other agencies and healthcare professionals to coordinate comprehensive care and support for patients
• Provide timely and effective responses to crisis situations, including suicide risk assessments and the development of appropriate intervention plans and referrals
• Guide patients in accessing community resources, acting as an advocate and liaison for patients and their families
• Stay informed about local and regional cancer resources to enhance patient support and care
