Summary

Join our team as a Workflow Consultant and help enhance physician and patient quality of life through Elation, our cloud-based clinical platform. You will advise primary care practices on clinical and operational workflows, focusing on team-based care, care coordination, and population health strategies. Design and optimize workflows to support Value-Based Care models. Collaborate with teams to ensure customer needs are met and value is achieved. Drive adoption and change management, proactively identify workflow opportunities, and mitigate implementation risks. This remote position requires experience in Value-Based Care programs and workflows.

Requirements

  • 3–5 years of relevant experience in clinical operations, healthcare consulting, or health IT implementation, preferably with a focus on primary care and/or value-based care
  • Deep understanding of primary care delivery models and experience with clinical workflows such as chronic disease management, preventive care, and team-based care
  • Proven experience supporting VBC programs (e.g., ACOs, MIPS, HEDIS, CPC+), including quality measure reporting and population health strategies
  • Ability to translate complex clinical requirements into practical, scalable workflows for diverse customer settings
  • Successful track record of identifying, designing, and implementing process improvements
  • Ability to communicate complex clinical workflows to both customers and internal stakeholders
  • Hands-on, high-energy, passionate, and creative problem-solver with the ability to get things done and lead others to success
  • Proficiency with AI tools and healthcare analytics to enhance efficiency and the customer experience geared towards outcomes

Responsibilities

  • Advise primary care practices on clinical and operational workflows, with emphasis on team-based care, care coordination, and population health strategies
  • Design and optimize workflows that support Value-Based Care models, including risk stratification, population health management, care gap closure, and quality measure reporting
  • Collaborate with front-line teams to drive customer-specific configuration design to meet workflow requirements and ensure value achievement
  • Drive adoption and change management by providing actionable, user-friendly solutions that improve outcomes and align with practice goals
  • Proactively identify workflow opportunities and operational improvements; make solution recommendations to client and project leadership
  • Mitigate implementation risks and issues with creative problem solving
  • Drive cross-functional collaboration with internal stakeholders to identify and propose product enhancements, workflow efficiencies, and customer engagement strategies
