Summary

The job is for a Care Coordinator at Pomelo Care, a multi-disciplinary team working on improving care for pregnant people and babies. The role involves supporting care navigation, filling out patient data, monitoring member support phone line, completing eligibility checks, developing new workflows, and reporting to the Market Operations Lead.

Requirements

• 4+ years healthcare administration experience, ideally within virtual care
• Familiarity with SDOH platforms and referrals

Responsibilities

• Support care navigation for patients needing help finding providers in their network or being connected to other care services
• Fill out patient data in our EHR from continuity of care documents and questionnaires
• Monitor our member support phone line and answer any questions that patients call in with
• Complete eligibility checks for new and existing patients and file claims
• Develop new workflows focused on improving care coordinator efficiency
