Join our team as a Care Coordinator to support patients needing mental health care outside our platform. You will act as a key connector between patients, providers, and external healthcare organizations, ensuring smooth transitions to appropriate care levels. Manage care coordination tasks in a fast-paced environment, helping patients navigate complex care decisions with empathy and professionalism. Communicate directly with patients, providers, and insurance representatives to maintain care continuity. Collaborate with a dedicated team focused on teamwork and operational excellence. This role requires experience in customer-facing or care coordination roles, along with strong systems fluency and the ability to handle high-volume workflows. Requirements
- 2+ years of professional experience in customer-facing or care coordination roles
- 1+ years of external-facing customer support experience
- Strong systems fluency with the ability to quickly learn and adapt to new platforms
- Ability to recognize signs of patient crisis and escalate appropriately
- Experience working in a high-volume, fast-paced environment
- Proven ability to manage tasks within a queue- and SLA-driven workflow
- Proficient in navigating CRMs, EMRs, and documentation tools
- Experience collaborating cross-functionally with clinical, partnerships, and operations teams in a remote setting
Responsibilities
- Support patients who need mental health care outside of our platform
- Serve as a key connector between Rula patients, their providers, and external healthcare organizations- ensuring smooth transitions to the right level of care, such as intensive outpatient programs, partial hospitalization programs, or specialty psychiatry
- Manage care coordination tasks in a fast-paced, detail-oriented environment
- Play a critical role in helping patients navigate complex care decisions, while maintaining empathy and professionalism in all communications
- This includes speaking directly with patients, external providers, and insurance representatives to ensure continuity of care
Preferred Qualifications
- Experience with Zendesk and Salesforce
- Experience working in a clinical or healthcare environment
- Background in HIPAA-compliant documentation and handling of sensitive health information
- Demonstrated cultural humility and experience serving diverse, underserved, or vulnerable communities
