Summary

Join Thyme Care as a Care Partner and guide cancer patients through their journey. You will be responsible for contacting members via phone, text, video, and email, providing support with resources and scheduling appointments. The role requires strong communication and empathy, along with comfort using technology and working remotely. After three months, you will be proficient in Thyme Care systems and meet performance metrics. This is a full-time, non-exempt position with a pay rate of $24.03/hour, working a 2:30 pm - 11:00 pm EST shift. Thyme Care is committed to diversity and inclusion.

Requirements

  • A member-first approach and passion for assisting people and their families during challenging times
  • Bias to action; ability to identify and prioritize needs and act on urgent and important needs immediately
  • Humility and proactive seeking of feedback from others; willingness to learn and grow
  • Effective listening and communication skills; ability to build rapport with members and colleagues
  • Proven track record of success in scaling businesses, fast-paced environments, and/or startups; comfort with rapid changes
  • Experience with video chatting, Google Suite, Slack, electronic health records, or comfort learning new technology
  • Experience working in a remote role in a quiet and secure environment
  • Ability to test internet speed prior to starting to ensure applications function as expected
  • Located within the lower 48 United States due to contractual limitations with accessing PHIs

Responsibilities

  • Talk to, navigate, and guide members who have cancer throughout their journey
  • Reach out to members by phone, text, video, and email to check in on their needs
  • Support members connecting with providers or resources (e.g., transportation, financial grants, emotional support resources, medical equipment)
  • Help members schedule health and wellness visits with the provider team
  • Conduct survey assessments to determine changes in their health, needs, goals, and values for advanced care planning
  • Complete at least 30 calls with members each day
  • Build strong, trusting relationships with members, prioritizing how to address their needs
  • Identify and prioritize a member's needs and connect them with healthcare providers, social services, diagnostic appointments, and medical treatments
  • Follow Care Team policies and procedures, escalation pathways, best practices, and highest standards
  • Be a valuable partner to members, guiding them to the right resources and information
  • Look for ways to improve the experience, quality, and outcomes of work with members
