Requirements

  • Current AHPRA Registration
  • Registered Nurse degree
  • Experience in Case Management and Aged Care
  • Highly developed communication and relationship management skills including strong documentation
  • Exceptional problem-solving skills and ability to relate with those navigating the Aged Care system
  • Demonstrated organisation and prioritisation skills
  • Agility and flexible in style
  • Can do attitude
  • A professional attitude and presentation
  • To be considered for employment with Respect, each candidate must satisfy the legislative requirements of Aged Care Worker Screening to ensure suitability to work in the aged care industry

Responsibilities

  • Manage client care plans, ensuring all services and resources are utilized to enhance client health outcomes
  • Conduct clinical assessments and provide hands-on care, along with clinical guidance for other team members
  • Serve as the primary contact for clients, families and advocates
  • Lead the onboarding process for new clients
  • Oversee planning, implementation and ongoing evaluation of care
  • Ensure all client services are delivered in line with Respect Aged Care’s values , organizational policies, compliance standards, and budget requirements

Preferred Qualifications

  • Experience managing caseloads of Home Care Package Clients with complex needs
  • Expertise in safety practices within the Aged Care or Health Sectors
  • Proficiency in conducting assessments, care planning, implementation, evaluation, and documentation of quality client care in line with Consumer Directed Care principles
  • Strong communication skills to liaise with staff, consumers, service providers, and health professionals, ensuring the delivery of consumer-focused services
