Summary

Join the Central California Alliance for Health as a Senior Complex Case Manager - Adult (RN)! We have six temporary positions available through April 2025. You will develop and manage individualized care plans for members, educate stakeholders, and participate in quality improvement studies. This role requires a current unrestricted California RN license, an Associate's degree in Nursing, and at least five years of patient care experience, including one year of case management. The Alliance offers a hybrid work environment and benefits including medical, dental, vision, paid time off, and retirement plans. While full Alliance benefits are not offered for this temporary position, a competitive compensation range is provided.

Requirements

  • Knowledge of: The principles and practices of clinical nursing
  • Knowledge of: The principles and practices of case management
  • Knowledge of: Care management and coordination
  • Knowledge of: Evidence-based practice guidelines in the development of care plans
  • Ability to: Demonstrate strong critical thinking and problem-solving skills
  • Ability to: Interpret and apply policies and regulations
  • Ability to: Define issues, conduct research, interpret data, and identify and evaluate options
  • Ability to: Evaluate medical records and other health care data
  • Ability to: Organize work and manage complex priorities
  • Current unrestricted license as a Registered Nurse issued by the State of California
  • Associateโ€™s degree in Nursing and a minimum of five years of experience in a patient care setting, including at least one year of case management experience; a combination of education and relevant work experience may be qualifying

Responsibilities

  • Develop and manage an individualized comprehensive plan of care for members referred into the assigned Case Management Program with the goal of promoting optimal, achievable outcomes in the most cost effective and appropriate manner
  • Work with and educate members, families, providers, external agencies and internal departments on the Case Management Program and providing assistance as needed
  • Participate in Quality Improvement studies, to continually evaluate the programโ€™s effectiveness and ability in promoting quality driven, cost effective, achievable goals and outcomes for members
