Summary

Join Aledade as the Director of Community Health Center Performance and develop, maintain, and utilize data systems to track contract performance for our CHC partners. Support performance management of CHC partners by identifying areas for improvement using data-driven insights. Leverage expertise in population health analytics and value-based care to lead the development of new programs and resources for CHC performance enhancement. Collaborate with internal and external stakeholders to develop strategies and solutions that support organizational key result (KR) performance outcomes. Work closely with other Aledade teams and present CHC performance information to CHC leaders. This role requires experience working with Federally Qualified Health Centers and a strong understanding of value-based care principles. The position offers flexible remote work within the US with occasional travel.

Requirements

  • Bachelorโ€™s degree and/or 10+ years of healthcare work experience
  • 2+ years experience working with Federally Qualified Health Centers or in a Federally Qualified Health Center
  • 5+ years of progressive value-based care experience
  • Demonstrated ability with population health analytics, including data systems and data visualization
  • Able to thrive in a fast-paced, startup environment
  • Demonstrated experience with taking an idea from concept to execution
  • Proven ability to successfully manage multiple programs, coordinating and collaborating across projects and teams
  • Strong and clear thinker/writer

Responsibilities

  • Collaborate with stakeholders to track performance (affordability, risk, quality, pop health impact) for CHCs on ACO performance, including presenting information at internal Steering Committees
  • Develop and manage CHC data dashboards with support from BI, and create visibility on CHC metrics
  • Leverage data insights to develop and execute on strategies to ensure VBC success via the CHC Center of Excellence
  • Support payer strategies designed for CHCs based on data
  • Participate in Collaborative Learning Networks, including the development of standard curriculums
  • Represent Aledade to CHCs during CHC summits and events
  • Other duties as assigned to ensure maximal coordination across Aledade teams

Preferred Qualifications

  • Expertise in FQHC business models, such as billing/coding strategies and FQHC operations
  • Leadership level experience in FQHC strategy and population health initiatives
  • Knowledge/background participating or leading IHI-style collaboratives
  • Skilled with Tableau
  • Masterโ€™s degree or other advanced degrees in business, healthcare administration, or public policy
