Requirements

• RN license in good standing
• Endless empathy for people, and a strong ability to fight for those who can’t
• Strong clinical skills with strong organizational skills to match
• Pride in your technical savvy. You have the ability to quickly and fluently learn new systems and softwares
• Extreme bias toward action and execution
• Fearless feedback. You care about forging the system that empowers better patients outcomes, and you’re not shy about sharing your thoughts

Responsibilities

• Learn the Solace systems, tools, technology, partners, and expectations
• Build strong, trusting relationships with Medicare patients
• Be able to identify and prioritize Medicare patients' needs and assist them to maintain a streamlined care continuum
• Develop patient care plans to holistically address social determinants of health
• Build the systems of the future in working with Medicare patients
