Telehealth - Injury Triage Consultant

Summary

Join Cogent Thinking, a leader in injury prevention and care coordination, as a full-time Triage Injury Management Consultant. In this role, you will connect with injured employees and their managers, providing injury triage, first aid advice, and support. You'll coordinate support services and resources, utilizing your clinical experience in a corporate setting. The work is primarily telephonic, allowing for flexibility between home and office. Requirements

• Experienced Allied Health (Physio, Osteo, Chiro, OT, EP etc) practitioner with full registration or a Registered Nurse
• Caring and empathic nature
• Well-developed written and verbal communication skills
• Good IT and time management skills
• Strong customer service focus

Responsibilities

• Support and care for clients with empathy and expertise to improve their personal health and wellbeing goals
• Undertake coordination of supports and wellbeing services for a highly engaged and motivated client population
• Provide initial early intervention Triage with clients, supporting both physical and mental health services
• Undertaking of relevant case coordination activity including liaison with all stakeholders; provision of email updates; and creation of appropriate documentation such as a treatment plans or other documentation as required

Preferred Qualifications

Experience care coordination or occupational rehabilitation
