Clinical Triage RN

Summary

Join our care model as a Triage RN, providing holistic, patient-centered care to patients with complex medical, behavioral health, and social needs. You will be part of the central urgent care team, caring for patients virtually via video, telephone, or text. Responsibilities include clinical triage, virtual assessments, and close care coordination.

Requirements

• Active, unrestricted Registered Nurse license in home state
• Willingness and certification in good standing to get licensed in requested states such as Michigan, Colorado, Indiana within 90 days of hire date
• 3+ years of experience providing clinical services to Adult and/or Geriatric individuals with co-occurring chronic medical and behavioral health conditions, particularly in virtual settings
• Possess exceptional clinical triage, clinical assessment skills and critical thinking skills

Responsibilities

• Perform clinical triage for inbound outreach from our patients to ensure the highest level of patient experience, safety, and connection to appropriate resources that can keep them safely at home and out of the emergency department/hospital when possible
• Care for patients primarily virtually via video, telephone, or text by providing virtual assessments of clinical symptoms, urgent counseling, escalation to care teams when appropriate and provide close care coordination
• Collaborate closely with Accompany Health Advanced Practice Clinicians and physicians as well as external providers to ensure appropriate resolution of triage issues
• Effectively interpret and utilize electronic data tools and analysis to organize daily activities and provide high quality of care
• Establish and foster trusting relationships with your patients and ensuring that care is appropriately aligned with their goals and values
• Provide feedback on program design and workflows to ensure we are providing the best patient care possible
• Ensure timely and appropriate documentation
