Community Service Coordinator

Summary

Join Clover, a company reinventing health insurance, as a Community Service Coordinator. You will provide administrative support to a care coordination team, assisting with tasks such as scheduling transportation, contacting providers, and helping members access community resources. This role involves coordinating clinical initiatives, providing administrative support to clinicians, and assisting members with benefit applications. The ideal candidate is passionate about helping people, highly organized, detail-oriented, and comfortable in a fast-paced environment. Experience with social service resources and applications is required, while bilingualism (English/Spanish) and healthcare experience are preferred.

Requirements

• Have a High School Diploma and/or GED
• Have excellent customer service and communication skills (verbal and written)
• Have strong administrative and computer skills, especially Google Apps (Mail, Calendar, Sheets, etc.)
• Have experience with social service resources and applications

Responsibilities

• Provide coordination assistance for ongoing and new clinical initiatives (e.g. medication adherence, readmission prevention) that has overlap/hand-off between the Nurse Practitioners and RN Care Managers
• Assist RN Care Managers with non-clinical follow-up tasks or care coordination (i.e. assisting with setting up transportation, contacting provider offices to confirm fax is received, contacting pharmacy to ensure refill was submitted, etc.)
• Provide admin support to clinicians (i.e. faxing, scanning, mailing information, etc.)
• Assist members with filling out applications for benefits like assistance programs and Medicaid

Preferred Qualifications

• Be bilingual in English/Spanish
• Have professional experience in Medicare, Medicaid, or health care settings

• A reasonable estimate of the base salary range for this role is $28,800 to $44,000
